Provider, Pharmacy & Formulary Search
As a member of Dignity Health Plan you are entitled to receive a list of all providers (Provider Directory) and pharmacies (Pharmacy Directory) that are in the network. You are also entitled to receive a list of covered drugs (Formulary). If you would like a formulary, provider or pharmacy directory mailed to you, you may call Member Services at 1-800-485-3793 or emailĀ info@dignityhealthplan.com.
